  • Manifests itself before the age of 22
  • Long-term
  • Substantial Functional Deficits

 

How do I go about finding out if someone’s eligible?

  • Psychological Evaluation With:

-IQ Testing

-Adaptive Behavior Scale

  • Psychosocial Evaluation
  • Medical Evaluation Documenting The Diagnosis

 

Range of Services:

  • Family Support Services
  • Health Care; Mental Health; Rehabilitation
  • Residential Services
  • Day And Employment Services

  • Be prepared before you call

–       Know what you “need”

–       Have a copy of the DDP2 with you

–       Have eligibility documents ready

  • Be reasonably patient
  • Advocate for yourself if you can’t make it to a session
  • Document everything!!

 

What are Family Support Services and who can get them?

 

 

 

Do I need a Case Manager?

  • Family Support Funded Service Coordination
  • Medicaid Service Coordination
  • Plan of Care Support Services

 

The Relatively Easy Stuff:

–       Parent training

–       Behavior Management

–       Short Term Skill Training

–       Parent Support Groups

–       Crisis Intervention

–       Educational Advocacy

–       Family Reimbursement, Emergency Respite Funding

–       Family Support Funded Case Management

–       Et. AI.

–       Remember, there are things you don’t need to be in the DD system to get, e.g. Benefits Assistance, Other Types of Advocacy, Services Funded by Other Entities

 

May take a Little More Effort (Hybrid Funding).

–       In Home Respite

–       Recreation

–       After School Programs

–       Overnight Respite

Most Challenging Enrollment (waiver required):

  • What’s a Waiver Anyway?
  • What if the person doesn’t have Medicaid?
  • Oh, and by the way…the system may look totally different in a few years
  • You do not apply for “the waiver”, you apply for a “waiver service”.
  • The Medicaid waiver is a funding stream.
  • You cannot be in 2 waivers!

 

Examples of Waiver Services

  • Generally funded through the Waiver:

–       Individualized Residential Alternatives (IRA’s)

–       Day Habilitation

–       Community Habilitation

–       Consolidated Supports and Services (Self Determination)

–       Family Ed & Training

  • May be able to get via other funding sources:

–       Supported Employment (SEMP)

–       Prevoc Services

–       In Home Respite

–       Hourly and Free Standing Respite (Includes overnight respite and recreation)

  • May obtain if you are already enrolled in waiver:

–       Environmental MOD

–       Assistive Technology/Adaptive Equipment

–       Plan of Care Support Service (PCSS)

  • And a few others!
