Advocates provide supportive services for the following housing concerns: \nHomeless Prevention\nHousing Court Support and Workshops on Tenants’ Rights\nHousing Improvements\nDRIE/SCRIE Rent Freeze Applications & Repairs\nHome Finding\nNYCHA & NYC Housing Connect Lottery Applications\nProviding Multicultural\, Lifespan Services for People with Disabilities \nNote: We do not rent apartments and we do not provide financial assistance. \nAdapt Community Network\nADAPT Community Network’s Housing Assistance Services offers case management to families living in New York City with a household member who is OPWDD eligible. We assist your family in reaching your housing goals through advocacy\, information and providing referrals to resources. As part of the housing supports offered at ADAPT\, we work closely with Doorways to Independence (home modification program) and OPWDD Housing Subsidy programs. \nDoorways to Independence program assist persons with physical disabilities\, the elderly and medically frail with their home modification and adaptive equipment needs. Funding is available to cover the cost of wheelchair ramps\, stairlifts\, accessible bathrooms\, light weight strollers\, adaptations for those who are deaf or have limited hearing\, etc. Full project coordination is offered for eligible applicants\, including home assessments\, scope of work development\, obtaining vendor/contractor estimates and project management. \nHOUSING ASSISTANCE

In reality\, guardianship takes away the person’s rights to make all the decisions in their lives. Supported decision-making (SDM) is a well-recognized alternative\, enabling persons with I/DD to make their own decisions with the support of trusted persons in their lives. Now that the New York State Legislature has  passed a bill recognizing SDM\, people with I/DD who go through an SDMNY facilitation process and develop Supported Decision-Making Agreements will be protected from discrimination based on their disability. \n\n\n\nSupported Decision-Making New York (SDMNY) began asa 6-year project funded by DDPC\, to pilot the use of SDM to divert persons at risk of guardianship\, and to restore rights to persons  already subject to guardianship. Now under a grant with OPWDD\, SDMNY has designed a model for delivering SDM facilitation services state-wide. \n\n\n\nThis presentation with SDMNY Associate Director Joan Cornachio\, and Jenna Lamm\, SDMNY Transition Specialist\, will explain how the SDMNY facilitation process works with people with I/DD\, to develop Supported Decision-Making Agreements (SDMAs) that may be used as a more empowering alternative to guardianship. To facilitate this mission\, the EFMNY offers online educational webinars through Zoom\, and community educational/awareness presentations. \n\n\n\nThese presentations include\, but are not limited to\, seizure recognition\, diagnosis\, treatment options\, first aid for the most common types of seizures\, and additional provided information including medical and social service resources. All training is customized according to each community organization’s request\, including optional additional training as needed. \n\n\n\nThis information about epilepsy will benefit everyone. For example\, the EFMNY training will provide participants with the information and tools needed to assist a person during a Seizure and afterward. \n\n\n\nEPILEPSY 101 – KNOW THE FACTS! \n\n\n\nEpilepsy occurs equally to persons of all ages\, races and socio-economic groups.   Most people with epilepsy develop seizures before the age of 21 or as an older adult.
