Presentation: Enrolling in Medicaid Through the Medicaid Waiver

Presenter: Elizabeth Carver, District Manager, Division of Fiscal and Administrative Solutions

Children ages 18 and under can enroll in Medicaid if enrolling in the HCBS Waiver. The income and the assets for the child only is considered. The application is processed through the local DDSO, approved for completeness and then taken to the 34th Street office for processing.

The necessary forms include the DOH 4220 application (blue and white), Supplemental Form A DOH 4495A , Disability Interview DSS-486 and the Medical Report for Determination of Disability (Must be original and not be older than one year) , Insurance form signed and completed by employer HR Department or Insurance Company if individual has supplemental insurance, and the Declaration of Support MAP2050A.

Also necessary:

– Birth certificate (if none, declaration of citizenship form)

– Double-Sided Copy of the SS card

– Proof of address ( copy of utilizing bill)

– Copy of LOC

– Current psychological

– Documentation of all reported income and assets

– Summary of Health Benefits, if there is supplemental health insurance

 

Ms. Carver requested that all documents be submitted in triplicate (original + 2 sets of copies)

Questions asked? Why are some applications returned?

Some people at Medicaid are asking for the parental income and assets to be included in the application even though it won’t be considered in order to get the Medicaid. It may also be returned if information is missing.

Ms. Carver also suggested that we write “OPWDD HCBS Waiver Application” on top of the application.

Ms. Carver mentioned that Medicaid is getting stricter about how quickly NODs for waiver get in the system after Medicaid is approved. Since you have to wait for a Medicaid number before MSC can be approved, this slows down the process of acquiring an NOD. She suggested using state funding for case management that’s available for up to 90 days. She acknowledged that it is not used very much in the City and that there’s a different process for applying for this but that it may be worth it for agencies to do. You must get a “State Funding Authorization Request”. She wasn’t sure what the process and regulations were, but that we should contact our SDIS or DDSO Liaison to find out. 

How soon will someone get Medicaid?

The Revenue Support Staff Member gets a receipt of delivery from HRA. That person waits one month. If there’s no Medicaid number by then, they will check weekly for Medicaid number. Once Medicaid is approved, they will e-mail the Medicaid number to the OPWDD staff person who submitted the application.

Agencies should advocate for the child if you submitted the application. If you haven’t heard anything in three months, you need to call and ask what is going on

Once Medicaid is noted in TABS system, the Revenue and Reimbursement Staff Person will wait two weeks and then send another e-mail to the OPWDD waiver coordinator asking about the NOD date. If the NOD is not entered by the 4th week, the Waiver Coordinator will be notified that an NOD is required in 2 days time or they will inform HRA to close the Medicaid case. If an NOD is not issued in time, OPWDD will owe Medicaid money.
