Manhattan Transition Meeting Student Information Exchange

    • Was held at Morton Street on January 23rd, 2013 and it was a huge success
    • Families were present as well as Transition Coordinators and Providers
    • The meeting began with an introduction and overview of the Manhattan DD Council and Transition Committee.

    Manhattan Transitional Committee

Is a sub-committee of the Manhattan DD Council, which generally meets on the 2nd Wednesday of every month at Life Adjustment Center, 1430 Broadway, suite 503. The purpose of the Manhattan Transitional Meetings is to assist in the transition process for individuals with Developmental Disabilities that are aging out of the Department of Education. Provider Agency’s are also a part of this Committee to assist in the process.

The purpose of today’s event is to give the opportunity for Adult Day Service Providers to come together with Transitional Coordinators to exchange student referral packets. It’s also a time for the students and families to speak to Adult Day Service Providers.

Parents should ask the Adult Day Service Providers what kinds of supports and services are offered: such as: Supportive Work, Day Habilitation Without Walls, Day Habilitation With Walls, etc. This way, as a parent you will know if the provider has the services that are appropriate for your son or daughter. Likewise, the Adult Day Service Providers should inform the schools, Transitional Coordinators, of the supports and services that are provided in addition to informing the families.

At today’s event, there were over 40 people present including about 12 students/family members.

Thank you all for attending and getting your packets ready in time.

See you all on March 13th, 2013

Next Meeting: March 13th, 2013, Life Adjustment Center, 1430 Broadway, Suite 503 (NOT 501),Corner of 40th and Broadway
