1. UCP/NYC

Housing Case Management services for individuals who are family support eligible with availability in all boroughs.

2. YAI

3. Lifespire

4. GHO

5. New York Services for the Handicapped / Camp Oakhurst

Summer Sleep-Away Camp

Children ages 8-19 and adults ages 20 and over with physical disabilities.

Planned out-of-home Respite

From September to June. Physically disabled children and adults with physical disabilities.

From June to August for children and young adults on the Autism Spectrum

6. Janet Davis / Metro DDSO

11 new programs are being funded this year including the expansion of New and Expanded Respite programs, Socialization Supports, Holiday Programs and Innovative Programs

The total allocation is $375,000.

Funding is subject to OSC approval. Please see the attached listing.

The Metro DDSO has available Family Reimbursement dollars. Contact 212.229.3118.

Discussions

• There was a brief discussion about an article that appeared in the NY Times regarding Abuses at the NYState Institution. There were hearings last Friday where people were asked to provide testimony on this topic.

• Linda Irby and Elly Rufer, Manhattan parents represented the Manhattan DD Council at the recent EPIC Conference in Lower Manhattan.

• We had a brief discussion on the Waiver Series that the Family Support Committee held this past year. The feedback was positive, particularly from provider staff that do not process waiver applications.

It was reinforced that if a family only gets either

1. Respite Waiver Program

2. SEMP or

3. General Family Support Services

they do not need to get on the Waiver. You can only get one category of service (like respite) before you enroll in the Medicaid Waiver. If you have an MSC you have to be in the Waiver to get additional services.

Projected date of the Fair: Thursday, December 1, 2011

Church of St. Paul the Apostle

We reviewed the budget and expenses from the 2011 Fair. The budget was approximately $10,000.

Topics for Workshops:

− Understanding the new IEP

− Future Care Planning

− Preparing for the Future of our Service System

− Travel Training

− Behavior Management

− Visual Communications workshop / sensory Integration workshop

− Housing Advocacy Workshop to include vendors

Information Table:

− We discussed that we need more publicity on the purpose of the Information Table

− We might consider having four people that wear a colored T-Shirt that states “How Can We Help You to Find A Service?”

− We need a strong person that will make sure we get evaluations returned

− We need to think of an incentive to keep the vendors to stay until 2pm
